You can make a referral by filling out the form below. Please read the information below carefully before submitting the form.

Consent

Please ensure that the Client has given consent for the information to be shared with other agencies for the purpose of assessing and addressing the clients support needs.

By completing this referral form you are supporting us to contact the client as safely and as quickly as possible. It would be appreciated if you could include as much information as possible – this saves the client from being asked the same questions twice and helps us to understand more about their needs, circumstances, and safety issues.

The referral process

We will contact the client/referrer to complete an assessment to determine safety and support needs.

• We may also need to contact other agencies that have been involved with the client’s support.
• Where risk factors have been identified a further risk assessment will take place with the client, and any additional information needed will be obtained.
• We aim to decide an outcome within 24-48 hours. For referrals received outside of our opening hours (10am – 5pm Monday to Friday) we aim to decide by the end of the following working day.

If there is a delay due to factors outside of our control you will be kept informed e.g. there may be delays in receiving information from other agencies involved.

We will do our utmost to ensure that all referrals are processed as quickly and efficiently as possible as it is important that we get the best support for the client.
